How you make a claim depends on how much your claim is likely to be.
Less than $1000…
No prior approval is needed but please read the schedule of benefits for your plan and our general terms and conditions to ensure the treatment or procedure you’re having is covered by us.
To make a claim under $1000 please fill in a claim form and post it to us along with your original receipts (sorry we don’t take photocopies).
$1000 or more…
We need to pre-approve the claim before the event. You need to:
- Fill in a prior approval form (your medical specialist will also have to fill in part of this form)
- Ask your doctor to fill in a medical report
- Send or fax your prior approval form and medical report to us.
It takes at least five working days for us to do any necessary checks before we can approve the claim, so it’s really important to get your pre-approval form and medical report to us as soon as you can.
We’ll let you know in writing if your treatment or procedure has been approved for a claim.
